Dental, Vision, Life/AD&D – a variety of products with benefit options to suit almost any need.

Provider networks for dental and vision are one more reason to choose BEST! Dental - Members have access to Dentemax Network www.dentemax.com.

Vision (new - Eyemed discount network!) - offers freedom of choice, but now includes Eyemed as a nationwide vision care discount network. Ask for a summary of discounts.  

On-line quoting. Quotes available same-day for groups of 2-149. (10+ Employer-contributory life quotes are run by Home Office.)

DENTAL – Indemnity Plus and Dental PPO Plans. Rates available in a proposal.

BEST Life Indemnity Plus Dental Plans are designed for groups with 2 or more employees, offers freedom of choice, can visit any dentist.

Voluntary Dental – if group cannot meet the participation requirements of Employer-contributory plans, it may qualify for the voluntary option. Requires greater of 5 employees or 20% of all eligible employees enrolled.

Value added special benefits – Children’s Good Vision Benefit (child vision exam-only benefit, to groups that elect ortho) and Special Supplemental Accident Benefit (covers injury to sound, natural teeth, pays 50% of usual, customary and reasonable.)

BEST Life PPO Dental offers freedom of choice, but enjoy the greatest level of savings within the network. Members have access to Dentemax national dental network and other regional networks. Accessing these networks will reduce the members’ out of pocket costs.

Multiple plan discounts – bundle 2, $ave on 1. Save 5% on dental plan rates when you bundle with vision or life. Similarly, bundle 3, $ave on 2. Save 5% on dental plan and vision plan rates when group purchases dental, vision and life.

Periodontics and endontics are available as either Class II (Basic) or Class III (Major) services in the Mid and High Plans – no group size restrictions! Get 90th  (UCR) percentile coverage – on all plans and for all group sizes! Get ortho on all plans and for all group sizes! Benefit available to enrolled dependent children through age 20. All groups eligible for plans featuring higher dental maximums (up to $2500) and greater coinsurance (100/90/60). Deductible options for all groups : $0, $25, $50, $75 and $100.

Rate guarantee. 12 month standard. 24 month available for Employer-contributory groups of 5 or more enrolled; Voluntary groups with 5-50 enrolled.

Minimum Participation: Employer-contributory plans (2-4): 100% participation; (5+): minimum of 5 employees enrolling & 60% participation; Voluntary (5+): minimum of 5 employees enrolling & 20% participation. Employees with other group dental coverage do not count toward participation requirements. Dependent participation is not required.

Takeover (or waiver of waiting period) - Employer-contributory plans. 12 month waiting period on perio, endo, and all Class III & IV services will be waived for groups of 5 to 9  enrolled employees with comparable coverage, and 10 or more enrolled with no prior coverage!

Takeover – Voluntary plans. 12 month wait on perio, endo and all Class III & IV services is waived for groups of 5-9 enrolled employees with proof of comparable coverage, 10-24 enrolled with 50% participation and comparable coverage, and 25 or more enrolled with no prior coverage! Ask for rules for groups of 10-24 with less than 50% participation.

Late entrant provision does not apply during open enrollment for all groups.

BEST Flex Group Life (for groups with 2-9 employees)

Supported by BEST Dental or Vision for groups under 10 enrolled. No ineligible industries, no additional administration fee. Guarantee issue for all group sizes. Available with Dependent Life.  Rates available in a proposal or premium grid. AD&D included in rates.

BEST Essential Life (for groups with 10 or more employees)

Stand Alone or with any other coverage offered by BEST Life. Guarantee Issue amounts are based on total group volume (ranging from $50k - $400k). Discounts are given to certain preferred industries. AD&D, Dependent Life and Supplemental Life are optional add-on plans. 

BEST Passport Life – Voluntary (for groups of 10 or more employees).  Portable. Book rates available.

BEST Vision (for groups of 2-9 when purchased with dental,  5 or more stand alone. Voluntary option – minimum 10 enrolled.)
Four plans to chose from, 3 different deductible options, contacts may be covered in lieu of frames and lenses or in addition to frames and lenses. New 2009 - Eyemed discount network! - offers freedom of choice, but now includes Eyemed as a nationwide vision care discount network. This program offers discounts for exams, lens options and add–ons (such as scratch resistance and UV coatings), contact lens and even laser vision correction with no limitations on how often these discounts can be obtained. These discounts are only available through EyeMed’s network, which has access to over 21,000 locations nationally and include many leading optical retailers including Lenscrafters, Target Optical, Sears Optical, JC Penny Optical and most Pearle Vision locations. Value-Added Discounts (EyeMed & QualSight) (for groups effective 2009 and onward).

Laser Eye Surgery Discounts – All BEST Life plans include 40-50% off the national average charge for laser eye surgery procedures administered by QualSight.  BEST Life members are automatically eligible to receive discounts and can do so by calling QualSight at 1-877-405-4448 before seeing any LASIK provider. More information is located in this attachment and on QualSight’s website.
